New Patient Priority with No Insurance (SELF-PAY) This is an out-of-pocket custom convenience service. You have the option to schedule an appointment during regular office hours by calling 561-559-1110 or filling out new patient registration. Insurance is recommended for medications.
Cancellation Policy
Cancellation & Rescheduling Policy Lab Appointments We kindly request at least 12 hours’ notice to reschedule lab appointments. Cancellations or changes made with less notice may incur a fee. Functional Testing To reschedule functional tests, please provide: • 48+ hours’ notice – no fee • 24–48 hours’ notice – 50% fee • Less than 24 hours’ notice or no-show – full fee applies Priority Appointments Priority appointments are non-refundable. However, we’re happy to reschedule your visit to another priority time with at least 24 hours’ notice. Cancellations made within 24 hours or missed appointments will be charged in full.
